Summary

Studies with high levels of evidence are needed to create algorithms specific for the man- agement of pediatric pneumothorax. Recommendations of recent adult guidelines on pneumothorax can- not be applied directly to children. Pneumothorax is rare before puberty, and most pneumothoraces are secondary in this age group. An exhaustive workup for the causing condition is necessary. Recurrence rates of children are higher than adults. Surgical pleurodesis seems highly effective and chemical pleu- rodesis is rarely used. Recently, catheters with smaller diameters are being used more often and these are of particular use for pneumothorax aspiration in children. Thoracoscopic apical wedge resection and pleurodesis has become the preferred surgical approach for children with recurrent, bilateral pneumoth- orax or lasting air leak. Pneumomediastinum almost always regresses with elimination of the causative pathology. In case the mediastinal pressure becomes too high and impairs cardiac functions, puncture and drainage can be done under ultrasound vision.
