Özet

disease of young men affecting their quality of life and functional

level that may necessitate total knee arthroplasties (TKA) at a relatively

young age. However, these patients learn to cope with their

functional deficits resulting from multiple joint involvements. The

aim of this study was to investigate the impact of TKA on patient-reported

outcomes of hemophilic arthropathy patients.

Material and Methods: In this retrospective study, 45 primary

TKAs of 30 hemophilic patients (mean age at the time of operation

41.62 ± 13.38), performed between 2002-2018

that were available

for final follow-up

were included. Fifteen patients had bilateral

TKAs, six were simultaneously performed. Mean follow-up

was 5.27 ± 4.53 years (range, 1-17).

Preoperative and postoperative

range of motion values (ROM), flexion contractures, Knee injury

and Osteoarthritis Outcome Scores (KOOS) with its subscales

(pain, symptoms, function in daily living, function in sport and recreation

and knee-related

quality of life), Knee Society (KSS) Knee

& Function Scores, visual analogue scale (VAS) for pain were compared.

Preoperative and postoperative bleeding complaints were

questioned. Statistical analyses were performed utilizing SPSS v18.

Significance level was set at 0.05.

Results: Contralateral knees of only three patients were asymptomatic;

29 out of 30 patients had multiple joint involvements.

Mean ± SD, median(minimum & maximum) values of ROM, flexion

contractures, KOOS, KSS, and VAS scores are presented in Table 1. All parameters were significantly improved postoperatively.

23 knees had preoperative bleeding episodes, while six of them continued

postoperatively. Five knees which were not bleeding preoperatively

have started during the follow-up;

34 out of 45 knees were

devoid of bleeding.

Conclusions: Hemophilic arthropathy of knee, a fairly common complication

of a fairly uncommon disease, is a debilitating condition

which may be reversed successfully with TKAs. Management of the

knee with TKA results in improvement of both objective and subjective

outcome measures in mid-term

follow-up.
