Introduction Mepolizumab is a targeted anti-interleukin-5 (IL-5) monoclonal antibody used to treat type 2 inflammatory diseases. Aims and objectives To study mepolizumab effectiveness in patients with severe asthma with high total IgE or CRSwNP. Methods NEST was a self-controlled cohort study of adults with severe asthma in Asia, Latin America and the Middle East who received ≥1 dose mepolizumab 100mg for 12 months (post-initiation period; N=525; Al-Lehebi RO et al. Adv Ther. 2024;41:4008–31). This was a post-hoc analysis of patients with IgE ≥76 UI/mL3 or CRSwNP. Endpoints: incident rate ratio (IRR) of clinically significant exacerbations (CSE; defined in Table), oral corticosteroid (OCS) use and dose and Asthma Control Test (ACT) score pre-/post-initiation. Results Patients with IgE ≥76 UI/mL3 (n=189) had a significant reduction in CSEs (79%; IRR [95% confidence interval (CI)] 0.21 [0.14–0.30]) and a reduction in OCS use and dose. The number of patients with well-controlled asthma was higher post-initiation (Table). In patients with CRSwNP (n=115) there was a significant reduction in CSEs (90%; IRR [95% CI] 0.10 [0.06–0.18]), and a reduction in OCS use and dose post-initiation (Table). Conclusions Inhibition of IL-5 by mepolizumab led to a reduction CSEs and OCS use in patients with severe asthma with IgE ≥76 UI/mL3 or CRSwNP. erj;66/suppl_69/PA5820/F1 F1 F1
