• güncel gastroenteroloji 21/125TEDAVİGÖRH olgularında, medikal tedavi ile yaklaşık 80 nüks gözlendiğinden, kür şansı düşüktür. Bu nedenle tedavideki amaç, semptomların ortadan kaldırılması, özofajitin iyileş-tirilmesi ve komplikasyonlardan korunma (striktür, Barrett özofagusu v.b.) olmalıdır (4). Bu tedaviler şöyle gruplandırılabilir:1. Yaşam tarzında değişiklikler (sosyal tedbirler) 2. Medikal tedaviler3. Endoskopik tedaviler4. Cerrahi tedavilerSosyal TedbirlerSigara içilmesi, miktara bağımlı olarak GÖRH semptomları-nın gelişimi için bir risk faktörüdür. Olgulardan sigarayı bırak-maları istenmelidir (5). Yine obezitenin GÖRH için önemli bir risk faktörü olduğu kanıtlanmıştır, bu nedenle aşırı kilo-lu GÖRH’lü olgulara kilo vermeleri önerilmelidir (6). Aşırı fiziksel aktivite, GÖRH gelişimi için önemli bir risk faktörü olmasına karşın düzenli hafif-orta düzeyde fiziksel aktivitesi olan bireylerde reflü semptomları daha az görülmektedir. TANIM ve TANIMLAMABaşta asit olmak üzere, pepsin, safra, pankreas sıvısı gibi mide içeriğinin zorlanma olmaksızın özofagusa geçmesi yani “gastroözofageal reflü”, fizyolojik bir olay olmasına karşın, özofagusa geçen materyal ile oluşan, hastanın yaşam kalite-sini bozabilen semptomlar ve/veya bulgular “gastroözofageal reflü hastalığı” (GÖRH) olarak tanımlanmaktadır (1). Gast-roözofageal reflüden koruyucu ilk bariyer olan alt özofagus sfinkterinin (AÖS), reflü olan materyalin kleransını sağlayan özofagus motilitesinin, üst özofagus sfinkterinin ve reflüjenik materyalin dilüe olmasını sağlayan tükrük içeriğinin herhangi biri ya da birkaçındaki fonksiyonel yetersizlik, yine mide bo-şalımının gecikmesi GÖRH etyopatogenezinde rol oynayan mekanizmalar olarak bilinmektedir. Bununla birlikte günü-müzde GÖRH etyopatogenezinde rol oynayan en önemli et-kenin geçici alt özofagus sfinkter relaksasyonu (GAÖSR) ol-duğuna inanılmaktadır . GAÖSR’nunu yutma sırasındaki AÖS relaksasyonundan ayıran en önemli 2 özellik faringeal kont-raksiyon ve özofageal peristaltizm oluşmadan ortaya çıkması ve relaksasyon süresinin daha uzun (10 saniye) olmasıdır.
