Summary

Examination of Life Experiences of Acute Leukemia Patients According to Roy Adaptation Model and Mishel Uncertainty Theory: Mixed Model

Objective: It is the examination of the life experiences of acute leukemia patients by determining the levels of uncertainty and adaptation in the disease, guided by the Roy Adaptation Model and Mishel Uncertainty Theory.

Method: The study was conducted as a mixed method research between March 2021 and March 2022 at Dokuz Eylül University Faculty of Medicine Hospital and Medical Park (Medical Point) Hospital Hematology and Stem Cell Clinic. In the quantitative part of the research, “G. Power-3.1.9.2” statistical program was used and the study was conducted with 96 patients and in the quantitative section with 15 acute leukemia patients. Quantitative department data were collected using the Patient Diagnostic Form, Coping and Adaptation Process Scale, and Mishel Uncertainty in Illness Scale. Qualitative department data were collected with the Patient Diagnosis Form and Semi-Structured Questionnaire. Quantitative section data were evaluated using SPSS 25, and qualitative section data were evaluated using Maxqda data analysis programs. Combination design, one of the mixed method research designs, was used to determine the design of the research.

Results: In the quantitative data of the study, the mean of disease uncertainty in leukemia patients was calculated as 16.22±3.39, and the mean of coping and adaptation was calculated as 117.17±10.62. It was determined that as the level of uncertainty of the patients increased, adaptation decreased and that a one-point increase in the level of uncertainty resulted in a decrease of -1.384 units in the level of coping and adaptation (beta = -1.384). The qualitative data of the study were coded in 5 categories: physiological domain-history of uncertainty, self-concept-evaluation of uncertainty, self-concept-coping with uncertainty, role function-history of uncertainty, interdependence-history of uncertainty.

Conclusion: Acute leukemia presents as a sudden onset, emotionally challenging, information-heavy condition with little time to make important decisions. It has been observed that the uncertainty level of acute leukemia patients is high and their adaptation and coping levels to the disease are inadequate, and as the level of uncertainty in the disease increases, adaptation decreases. Sources of uncertainty and coping situations in the disease are explained with qualitative data.

Keywords: leukemia; uncertainty, adaptation, cope with; nursing
