INFLAMMATORY PROFILE IN PATIENTS WITH ACROMEGALY AND HEPATOSTEATOSIS: A COMPARATIVE ANALYSIS OF NUCLEAR FACTOR KAPPA B (NF-κB), CYCLOOXYGENASE-2 (COX-2), MONOCYTE CHEMOATTRACTANT PROTEIN-1 (MCP-1), INTERLEUKIN-1β (IL-1β), AND TUMOR NECROSIS FACTOR-ALPHA (TNF-α) LEVELS

Summary

Purpose: Inflammation may link growth hormone (GH)/insulin-like growth factor-1 (IGF-1) excess to hepatic steatosis in acromegaly. This study compared circulating nuclear factor kappa B (NF-κB) p65, cyclooxygenase-2 (COX-2), monocyte chemoattractant protein-1 (MCP-1), interleukin-1β (IL-1β), and tumor necrosis factor-alpha (TNF-α) levels, together with the Fibrosis-4 (FIB-4) index, in patients with acromegaly versus healthy controls, and according to disease control status.

Methods: This cross-sectional case-control study enrolled 48 patients with acromegaly (28 controlled, 20 uncontrolled) and 30 healthy controls between 2023 and 2025 at the Department of Endocrinology, Ege University Faculty of Medicine. Anthropometric, biochemical, and inflammatory biomarker measurements were obtained and FIB-4 was calculated. Group comparisons, correlation analyses, logistic regression, and ROC analyses were performed.

Results: Patients with acromegaly were older and had higher fasting glucose, HbA1c, GH, IGF-1, and FIB-4 than controls (all p<0.05). Among inflammatory markers, only IL-1β differed between groups (p=0.030); NF-κB p65, COX-2, MCP-1, and TNF-α were comparable. IGF-1 correlated inversely with MCP-1 and IL-1β, and a strong positive correlation network emerged among inflammatory markers. Hypertension, HbA1c, IGF-1, and FIB-4 were independent factors associated with acromegaly. IGF-1 (AUC 0.752) and FIB-4 (AUC 0.686) showed significant discriminative performance (FDR-corrected). FIB-4 was higher in controlled than uncontrolled disease, whereas GH and IGF-1 were higher in uncontrolled acromegaly; inflammatory markers did not differ by control status.

Conclusion: Acromegaly is characterized by a higher FIB-4 index and an altered inflammatory correlation network, independent of individual cytokine concentrations. FIB-4 emerged as an independent factor associated with acromegaly, supporting its potential utility for non-invasive hepatic fibrosis risk assessment.
