THE EFFECT OF IMPLEMENTATION CENTRAL CATHETER CARE BUNDLE TO THE CATHETER ASSOCIATED INFECTIONS IN CHILDREN

Project no. 76 · Status: Kapanmış · 2016

Summary

Central catheter care bundle consists of a set of evidence based practices for improving outcomes of patients with central catheters. The aim of this quasi-experimental study is to investigate the effect of implementation central catheter care bundle to the catheter associated infections (CAI) in children. Seventy paediatric patient admitted to Paediatric Surgery Department of a university hospital, younger than age 18 years, having central venous catheter and who were allowed by their relatives to participate were included in the study. In this study central catheter bundle consists of “hand hygiene, catheter care, changing intravenous lines and minimizing enteries” parameters. The protocol of this study consisted of three phases. In the initial phase, observations were made to determine the central catheter care bundle adherence of healthcare professionals for 421 catheter-days. In the second phase, education was provided to 33 physician and nurses on the subject of the central catheter care bundle. For the third phase, the effect of central catheter care bundle adherence on the VAP rates after education was investigated. The study data were analyzed with the SPSS programme. It was detemined that the overall compliance with central catheter care bundle was determined as 32.4% (n:141) before education and 86.3% (n:623) after education. The physicians and nurses central catheter care bundle adherence after education showed statistically significant improvement (p: 0.0001). There were five CAI events in the pre-implementation period and three CAI events in the post-implementation period. It was determined that the number of SKIKDE decreased in the post-training period compared to the pre-training period, but this difference was not statistically significant (p: 0.207). In conclusion central catheter care bundle implementation with education prepared according to evidence based guidelines decreases the CAI rates. Therefore it is recommended implementation of central catheter care bundle on care of paediatric patients with central venous catheter. Key words: Central venous catheter, care bundle, infection
