Summary

Histiocytic and dendritic cell neoplasms are rare tumors with heterogeneous biological behavior and an unpredictable prognosis. This study aimed to investigate the prognostic impact of PDL1 expression in tumor cells and immune cells, BRAFV600E mutation status, tumor microenvironment features, and various clinicopathological parameters in histiocytic and dendritic cell neoplasms. A total of 108 cases of histiocytic and dendritic cell neoplasms diagnosed between 2006 and 2025 were included. Morphological findings, microenvironmental features, Ki67 proliferation index, BRAFV600E mutation status, and PDL1 expression were evaluated using cut-off values of 5%, 10%, 20%, and 50%. In addition, PDL1 expression in tumor cells was assessed by an artificial intelligence-assisted digital analysis method and compared with pathologist-based evaluation.

In univariate and multivariate analyses, a high Ki67 index, histiocytic atypia, elevated CRP and LDH levels, anemia, high ECOG performance score, advanced Ann Arbor stage, multisystem involvement, bone marrow involvement, and risk-organ involvement were associated with poor prognosis. PDL1 expression of ≥5% in tumor cells was associated with better overall survival, whereas PDL1 expression of ≥20% in immune cells was associated with poorer survival. In multivariate analysis, tumor cell PDL1 positivity was identified as an independent favorable prognostic factor, while immune cell PDL1 positivity and multisystem involvement were identified as independent adverse prognostic markers. In addition, a significant association was demonstrated between tumor cell PDL1 expression and BRAFV600E mutation. A strong correlation was observed between pathologist-based and artificial intelligence-assisted scoring methods (r = 0.99). Our findings suggest that PDL1 has prognostic and potential therapeutic significance in histiocytic and dendritic cell neoplasms.
