Summary

ABSTRACT

Background and Objective: Sarcopenia is a major geriatric syndrome characterized by a decline in muscle mass, muscle strength, and physical performance, leading to functional impairment in older adults. Cut-off values used in the diagnosis of sarcopenia are generally derived from different populations, and inter-population variability may affect diagnostic accuracy. The aim of this study was to determine reference cut-off values for appendicular skeletal muscle mass (ASM) and its derived indices using whole-body dual-energy X-ray absorptiometry (DEXA) in a healthy Turkish young adult population.

Methods: This cross-sectional study included 175 healthy volunteers (93 women, 82 men) aged 20–45 years without known chronic diseases between August 2022 and July 2025. Muscle mass was measured using DEXA; handgrip strength was assessed with a dynamometer, and physical performance was evaluated using the 4-meter gait speed test. Anthropometric measurements were obtained using standard methods. Statistical analyses included parametric and non-parametric tests, and cut-off values were calculated as 1, 2, and 2.5 standard deviations below the young adult mean.

Results: The mean age of participants was 29.85±7.41 years. Mean ASM was 13.65±2.00 kg in women and 22.48±2.87 kg in men. Handgrip strength was 27.76±3.96 kg in women and 46.07±6.98 kg in men, while gait speed was 1.19±0.15 m/s and 1.23±0.17 m/s, respectively. Strong positive correlations were observed between ASM and muscle strength as well as anthropometric parameters (p<0.001). The -2 SD cut-off values for ASM were 9.65 kg in women and 16.74 kg in men, while the -2.5 SD values were 8.65 kg and 15.3 kg, respectively.

Conclusion: This study provides DEXA-based reference and cut-off values for muscle mass in a Turkish young adult population. The findings support the importance of using population-specific reference values in the diagnosis of sarcopenia. In addition, muscle mass should be evaluated together with muscle strength and physical performance parameters. The -2.5 SD cut-off values may offer a more conservative and clinically useful approach for screening and diagnosis.
