Comparison of the treatment outcomes of arthrosynthesis and intra-articular injection materials in temporomandibular joint disorders: a randomized controlled study

Project no. 22038 · Status: Kapanmış · 2020

Summary

Aim: This project was aimed to evaluate the outcomes of injectable platelet-rich fibrin (i-PRF) combined with arthrocentesis in patients had temporomandibular joint disorders.

Materials and Methods: Patients suffered with pain and had a limited mandibular movements, with osteoarthritis or disc displacement without reduction were included in this project. Pain was recorded using the visual analog scale (VAS). Maximum mouth opening (MMO), lateral (contralateral and ipsilateral) and protrusive movements were measured using a milietric ruler. All variables were assessed preoperatively and then repeated at 1, 2, 3, 6, and 12 months postoperatively. Age, gender, duration of symptoms and affected temporomandibular joint were also recorded.

Results: Thirty-six patients with osteoarthritis (i-PRF group: 22 degenerative joints, 16 females/2 males; mean age 44,67 ± 12,13; arthrocentesis group: 21 degenerative joints, 17 females/1 male, mean age 45,72 ± 13,12) and 76 patients with disc displacement without reduction (i-PRF group: 43 degenerative joints, 34 female/4 males, mean age 47,2±9,1; arthrocentesis group: 44 degenerative joints, 35 females/3 males, mean age 46,8±10,2) were treated. There was a pain reduction and improvement of mandibular movements from the 1st to the 12th month postoperatively and the change in i-PRF groups was higher than the arthrocentesis group (p<0.001). In patients with disc displacement without reduction, treatment success in the i-PRF group was higher than the arthrocentesis group (100% and 73.7%, respectively; p=0,012).

Conclusion: Intra-articular injection of i-PRF combined with arthrocentesis might be an alternative for the patients which to reducing pain and improving mandibular function.
