Reducing secondary neonatal morbidity to hypoxic ischemic encephalopathy
Project no. 124 · Status: Kapanmış · 2017
Summary
Hypoxic ischemic encephalopathy is an important cause of permanent damage to the central nervous system and can still result in neonatal death, cerebral paralysis, and developmental retardation despite all improvements in neonatal intensive care practices. Perinatal asphyxia is the most common cause of death after premature birth (28%) and serious infection (26%). With this infrastructure project, it is aimed to provide therapeutic hypothermia device for infants with hypoxic ischemic encephalopathy and to reduce the mortality and secondary morbidity to HIE. Hypothermia treatment was applied to 6 patients who were diagnosed with HIE since the device was purchased with the hypothermia device gained with this project. In the patient group 83.3% were born by vaginal felivery. Four of the six patients (66.6%) had a difficult and prolonged birth history.Fifth minute Apgar score averages were 4.20 ± 2.77, cord blood gas or mean pH in the first hour was 6.95 ± 0.1 and base deficit was 17.68 ± 4.21. According to Sarnat and Sarnat scoring, patients were evaluated as stage 2 and 3 stage HIE. The patient, who had Stage 3 severe asphyxia and bleeding diathesis due to disseminated intravascular coagulation, died 20 th hour during the follow-up. One patient was transfered to pediatric surgery for esophageal atresia operation after hypothermia tratment. Long-term outpatient follow-up of four patients who are discharged with health still continues. With this advanced technology medical device provided to our neonatal intensive care unit, life-saving and long-term disabling treatment is offered to newborn babies diagnosed with HIE.
