Investigation of the Effects of Two Different Intracanal Medicaments Used in Regenerative Endodontics in Periapical Inflammation
Project no. 388 · Status: Kapanmış · 2018
Summary
The objective of the present study was to investigate the effects of Ca(OH)2 and double antibiotic paste (ciprofloxacin, metranidazole) (DAP) on levels of matrix metalloproteinase (MMP) -8 in periapical exudates during regenerative endodontic procedure. Twenty healthy patients (age range = 6–9 years) diagnosed with symptomatic irreversibl pulpitis with symptomatic apical periodontitis, immature mandibular first molar teeth, included and randomly divided two groups according to intracanal medication, Ca(OH)2 and DAP (ciprofloxacin, metranidazole, 1:1). Before application of intracanal medicament, periapical exudate samples were collected. Second sampling procedure performed at 14 days after first session, before intracanal bleeding process. MMP-8 levels were measured by Immunofluorometric assay (IFMA). National Institutes of Health Image-J program with TurboReg plug-in was used for standardization of the radiographs and to determine the increase in root length, root width and radiographic root area. All data were analyzed using by SPSS 25.0 computer software (all p<0,05). At the 12-month follow-up visit, all teeth showed sigificant increase in rooth lenght, root width and radiographic root area (p<0,001). Surprisingly, significant increase detected between beginning and final MMP-8 levels in 2 weeks (p<0,001). The effects of Ca(OH)2 and DAP medicaments on the periapical MMP-8 levels were found to be similar to each other (p=0,255). In conclusion, it was observed that the teeth treated with regenerative endodontic procedures increased in root length, thickened root walls and increased radiographic root areas. Both intracanal medications were effective in increasing the levels of MMP-8 in periapical region but there was not any significancy between the groups. Also, It was observed that MMP-8 acts as an anti-inflammatory enzyme during the healing of periapical pathology in open apices molars with symptomatic apical periodontitis.
