Summary
Type 2 diabetes mellitus (T2DM) has a large and increasing impact on global morbidity and mortality with microvascular, macrovascular and metabolic complications. The number of T2DM and associated diseases is now recorded as 23 years. Among patients with T2DM and insulin resistance the presence of substances was reported such as; E-selectin, P-selectin, intercellular adhesion molecule-1 (ICAM-1), Vascular cell adhesion molecule-1 (VCAM-1), interleukin-6 (IL-6), and Monocyte chemotactic protein (MCP-1). In chronic complications with T2DM; cognitive dysfunctions are also noteworthy. Patients with cognitive dysfunction may be in a spectrum ranging from mild cognitive impairment (defined as cognitive dysfunction without difficulty in performing daily activities) to severe dysfunction (dementia, Alzheimer). In this study, we aimed to determine the serum levels of VCAM-1, ICAM-1, P-selectin, FGF-21 and, APRIL in Type 2 diabetic patients with cognitive dysfunction and to determine their roles in the pathogenesis of T2DM-related cognitive dysfunction compared with Type 2 diabetic patients. Patients with type 2 diabetes mellitus who had dementia [(40)- 26F/14M] and non-dementia [(40) 33F/7M] who were admitted to the Ege University Internal Medicine Department polyclinic, Geriatrics polyclinic, Neurology polyclinic were included in our study. VCAM-1, ICAM-1, P-selectin, FGF-21 and APRIL levels of all cases were determined by ELISA. VCAM-1 levels were significantly 1,4 fold higher in patients with dementia (973.8 ± 650.7 ng/ml versus 699.4±129.6 ng/ml) and ICAM-1 levels were found to be increased 1.6-fold (578.1± 440.6 ng/ml versus 351.6±76.1 ng/ml) compared to the cases with diabetes. There was no significant difference in the P-selectin levels in the diabetic group with and without dementia. FGF-21 (450.1±286.1 pg/ml versus 316.4 ± 62.8 pg/ ml) and APRIL (17.7 ±10.3 ng/ml versus 13.7 ± 3.18 ng/ ml) levels were significantly increased 1.4 and 1.3 times in the dementia diabetic group, respectively. To understand the linkages between diabetes and dementia, it is thought that VCAM-1, ICAM-1, P-selectin, FGF-21 and APRIL levels should be investigated with further studies and it is possible to develop new strategies in the prevention and treatment of dementia.
