Project no. 28431 · Status: Kapanmış (Yayın bekleniyor) · 2023
Summary

Aim:

The aim of this research is to evaluate the effect of adipose tissue derived stem cell applications on endometrium regeneration, on thin endometrium rat model.

Material and Method:

In our study, eight-week-old Sprague-Dawley rats ( 43 female, non pregnant rats) weighing 200-250 g were used. The thin endometrium model was set up by injection anhydrous ethanol (95%, %0,5 ml) into the uterine cavity of rats. A preliminary experiment, 3 rats were sacrified, coagulation necrosis was proven. Forty rats were randomly divided into four groups: normal group (n=10) without intervention, systemic intervention group(n=10)

which IV injected stem cells ( 2.000.000 cells, 0,1 ml) into tail ven into thin endometrium, intrauterine intervention group(n=10) which intrauterine injected stem cells (2.000.000 cells, 0,1 ml) into both horns equally into thin endometrium, control group(n=10) which saline injected into thin endometrium. At the third estrus phase confirmed by vaginal smear after injection of stem cells, all rats were sacrificed under anesthesia, hysterectomy was performed.

HE staining was performed for evaluation of endometrial thickness and the number of endometrial glands.

Immunohistochemical evaluation was performed to analize of expressions of cytokeratin, vimentin, integrin αγβ3, LIF (leukaemia inhibitör factor) proteins in endometrial tissue.

The immunomodulatory effects of stem cells were evaluated by analyzing IL-1β, TNF α, bFGF, IL-6 mRNA expression.

Findings:

Fibrosis and inflamation response was the highest in control group than others. In control group, endometrial glands were few, fibrotic areas were detected in endometrium and myometrium layer, PMNL infiltrated between endometrial glands, connective tissue fibers increased. In both intervention groups, endometrial epitelium was found similar to the normal group.

By immunohistochemical analysis; cytokeratin and vimentin expression was found less in control group compared to others and was found similar in normal group, systemic and intrauterine intervention groups. There was no statistically significant difference integrin expression among all groups. LIF expression was detected prominently less in control group than others. LIF expression was detected similar between normal group and intrauterine

intervention group. In systemic intervention group LIF expression was detected less than normal group. Migration of stem cells into damaged endometrial tissue was proved by demonstrating the presence of stem cells in endometrial tissue labeled with BrdU before transfusion.

We detected no statistically significant difference among all groups about the gene expression of bFGF mRNA, IL-6 mRNA and TNF- α mRNA. Expression of IL-1β mRNA is up-regulated in intrauterine intervention group compare to normal group. IL-1β gene expression was found similar among normal group, contral group and systemic intervention group.

Result and Conclusion:

The migration of stem cells to damaged tissue, their regenerative effects have been proven immunohistochemically. No significant results have been found regarding the immunomodulatory effects of stem cells.

Disadvantages of stem cells such as low tissue adhesion, difficulty in preservation and transportation, lack of knowledge of the optimal treatment procedure, the possibility of causing malignancy limit their clinical use.

Alternative treatments such as secretome and exosome, which have high safety profiles and can preserve regenerative properties are worth investigating.
