Comparison of the growth in conventional cultures with the separated nuclear acid normal multiplex real-time polymerase chains test kit with a pre-diagnosis of septic shock

Project no. 23097 · Status: Kapanmış (Yayın bekleniyor) · 2021

Summary

Despite advances in sepsis management and targeted early treatment approaches, survival rates are still quite low. Mortality rates can rise from 20% in sepsis to 80% when septic shock develops. Considering the limitations of blood culture in the diagnosis of sepsis, the necessity of molecular rapid diagnostic tests comes to the fore. The aim of the study was to compare the growth in peripheral blood culture with the nucleic acid-based multiplex real-time polymerase chain reaction test kit and the growth in conventional blood culture in cases with a preliminary diagnosis of community/hospital-acquired septic shock.

Patients with a pre-diagnosis of community/hospital-acquired septic shock [According to Sepsis 3 definitions, patients with a lactate level > 2 mmol/L who require vasopressors to keep the mean arterial pressure ≥65 mmHg, who are consulted to the Department of Infectious Diseases and Clinical Microbiology, will be included (JAMA, 2016). )] 200 cases with similar demographic characteristics will be recorded based on patient files and electronic records. A case report form will be completed for each registered patient file and signed by the principal investigator or an authorized representative of the study staff. End of treatment, first week and first month survival are the dependent variables, demographic information, possible foci of infection, laboratory parameters (liver and kidney function tests, C-reactive protein, procalcitonin, hemogram), culture (blood, urine, sputum, cerebrospinal fluid, drain, tissue) and nucleic acid-based multiplex real-time polymerase chain reaction results to be studied from the whole blood sample taken at the 0th hour of septic shock diagnosis, empirically started antimicrobial drugs, and antibiotic change data are the independent variables. SPSS 25.0 will be used, measurement variables will be classified as mean±1S, and data will be presented with number-% tables. P<0.05 will be considered significant.
