Effects of Fluoride-containing Biomimetic Agents on Remineralization and Color Change of Enamel White Spot Lesions: An In Vitro Study
Project no. 23972 · Status: Kapanmış · 2022
Summary

In modern dentistry, the fundamental approach is to prevent the formation of dental

caries and to treat existing carious lesions using non-invasive or minimally invasive

approaches. Among the preferred strategies for implementing these approaches, the

use of remineralization agents stands out. Although widely used agents today are

considered successful, it has been reported in numerous studies that they provide

remineralization only in superficial tissues and are not succesful to reach deeper

tissues. Therefore, biomimetic remineralization agents synthesized in the laboratory

have been developed, with P11-4 oligopeptide being one of the leading materials. What

sets this material apart from others is its ability to facilitate remineralization all the

way to deeper tissues by forming a scaffold within the lesion body.

In this in vitro study, the effect of the combination of the gold standard, fluoride, with

biomimetic materials (P11-4) (P11-4F) on remineralization and color change was

compared. Fluoride-containing agents used in the treatment of white spot lesions were

employed to treat artificially created white spot lesions on bovine incisors (Fluoride

Varnish: Voco Profloroid Varnish (Cuxhaven, Germany) (5% NaF), CPP-ACFP: MI

Paste Plus TM with RecaldentTM (GC Cooperation, Tokyo, Japan), Self-Assembling

Peptide (P11-4 fluoride): CURODONT Repair Fluoride Plus TM (Credentis AG, US).

The remineralization capacities of these agents and their effects on color change were

evaluated at 4 time points using DIAGNOdent 2095, QLF-D Biluminator, SEM-EDS,

and VITA Easyshade®.

Significant differences were found among the four groups at 7th (T2) and 30th days

(T3) in the laser fluorescence evaluation with DIAGNOdent 2095 (p<0.05). On the 7th

day, the fluoride group was found to be more successful than the control group, while

on the 30th day, the CPP-ACFP group showed better improvement than the control

group (p<0.05). According to EDS results, after 30 days, the CPP-ACFP and P11-4F

groups significantly increased the Ca (% by weight), F (% by weight), and Ca/P ratio

compared to the fluoride varnish group (p<0.05). SEM images revealed that in the

groups where the remineralization agent was applied, the pores and tubules were

V

blocked, and the surface was covered, whereas these findings were not observed in the

control group. It was observed that applying a remineralization agent is a good option

for blocking pores and tubules. According to QLF-D Biluminator data, P11-4F

application provided more successful fluorescence changes compared to CPP-ACFP

and fluoride applications and was statistically more successful in reducing lesion

volume and maximum lesion depth (p<0.05). According to color change results,

applying a remineralization agent was found to be a statistically successful method in

restoring the tooth's original color (p<0.05). However, there was no significant

difference between the groups (p>0.05).
