Diyabetik Maküler Ödem Tedavisinde Subtreshold Laserin Etkilerinin Optik Koherens Tomografi Anjiyografi ile İncelenmesi

Project no. 132 · Status: Kapanmış · 2017

Summary

Purpose: To investigate the effects of subtreshold yellow pattern laser (SYPL) in the treatment of diabetic macular edema using optical coherence tomography angiography (OCTA). Methods: Type-2 diabetic patients who admitted to Ege University, Department of Ophthalmology, Retina Division between October 2018 and January 2020, and diagnosed as diabetic macular edema (DME) with central retinal thickness (CRT) <400µ using optical coherence tomography (OCT) were enrolled. All patients underwent complete ophthalmological examination, along with color fundus photography, OCT, and OCTA. SYPL treatment was performed to all patients by creating a treatment map from the patients’ fundus photographs and OCT images, and protecting foveal avascular zone (FAZ) area. Following SYPL treatment, examinations were repeated at 1st and 3rd month of treatment and compared with the baseline. Results: The mean age of 30 patients (15 female, 15 male) was 63.7±6.7 (range, 48-74) years, the mean disease duration was 17.9±5.4 (range, 13-27) years, and the mean HbA1C was 6.6±0.5 (range, 5.7-7.7) g/dL in the study population. The mean best-corrected visual acuity (BCVA) was 0.2±0.2 (range, 0-0.7) logMAR, 0.2±0.2 (range, 0-0.7) logMAR, and 0.2±0.2 (range, 0-0.7) logMAR (p=0.698); CRT was 323.7±40.1 (262-393) µm, 316.8±40.9 (268-377) µm, and 318.1±39.9 (226-396) µm (p=0,591) at the baseline, 1st month and 3rd month post-treatment, respectively. OCTA revealed the mean vessel density in superficial and capillary plexuses as 44.7±4.6 (range, 37.4-52.3), 45.6±4.7 (range, 38.6-54.9), 44.6±3.9 (range, 37.5-49.8); and 43.1±4.8 (range, 36.3-52.7), 45.3±4.8 (range, 38.9-54.2), 42.7±3.3 (range, 37.4-49.3), respectively (p=0.383 and p=0.291); and the mean FAZ area as 0.33±0.12 (range, 0.14-0.59) mm2, 0.32±0.10 (0.13-0.50) mm2, 0.33±0.09 (range, 0.14-0.44) mm2 (p=0.998) at the baseline, 1st month and 3rd month post-treatment, respectively. Conclusion: At the end of 3 months of follow-up, SYPL for the treatment of DME did not result in statistically significant change in BCVA, CRT, and macular capillary perfusion on OCTA evaluation.
