Comparison of Postoperative Pain and Clinical Success Following Shaping with Manual Instrumentation, Nickel-Titanium Rotary, and Reciprocating Systems in Teeth with Symptomatic Irreversible Pulpitis

Project no. 32255 · Status: Kapanmış · 2024

Summary

Patients presenting to the emergency dental clinic with acute pain and diagnosed with symptomatic irreversible pulpitis will undergo emergency treatment in the first session, which will include pulpectomy (removal of the coronal and radicular pulp) and root canal irrigation. This study aims to evaluate the impact of different file systems used for pulp extirpation and root canal shaping on postoperative pain levels—measured using the Numerical Rating Scale (NRS)—and on clinical success, based on patient follow-ups following emergency intervention in cases of severe pain.

For the removal of inflamed pulp tissue and root canal shaping, conventional hand files as well as two nickel-titanium (NiTi) rotary systems operating with reciprocating and continuous rotation motions will be used. One of these systems is the widely used WaveOne Gold file system, which functions with a reciprocating motion. The other is the newly developed ProTaper Ultimate file system, which operates with continuous rotation and has not yet been evaluated in clinical studies regarding its effect on pain.

In this study, the ProTaper Ultimate system will be used for the first time in the emergency treatment of patients presenting to the endodontic emergency clinic with acute pain, and who report a pain score of 7 or above on the Numerical Rating Scale (indicating severe pain requiring urgent intervention). The effects of three different shaping systems on postoperative pain and clinical success will be compared. Additionally, the study will contribute to determining the effectiveness of these three distinct root canal shaping principles—manual instrumentation, reciprocating motion, and rotary motion—on pain intensity in emergency endodontic cases by applying a clear and standardized treatment protocol. Ultimately, the findings are expected to contribute to the literature by providing a valuable dataset for endodontic emergency management.
