Effectiveness of Intravitreal Amniotic Membrane Extract Injection in Experimental Proliferative Vitreoretinopathy Model

Project no. 22606 · Status: Kapanmış · 2022

Summary

Proliferative vitreoretinopathy (PVR) is a fibrotic and inflammatory process that develops after retinal detachment, which is the separation of retinal layers, and is the most important factor that reduces surgical success. The main pathology in PVR is fibrotic membranes and inflammatory cytokines and growth factors. Amniotic membrane (AM); It is the inner layer of the placenta and has a matrix rich in cytokines and growth factors. These properties facilitate epithelial proliferation; It has healing effects by suppressing inflammation, scarring and angiogenesis. In eye diseases, AM is used by suturing in ocular surface diseases; However, this use requires surgery and its opaque structure prevents clinical examination. In order to reduce these limitations, the idea of preparing amniotic membrane extract (AME) has emerged. AME has similar features to AM. There is no study on the intraocular application of this extract. The aim is to evaluate, for the first time in the literature, the effectiveness of intravitreal AME application in preventing the development of PVR. It is envisaged that proliferation and fibrosis can be suppressed, and their development in the first place can be prevented by targeting the chemical nature of tractional membranes. A total of 32 rabbits will be included. For the PVR model, intravitreal 0.05U dispase injection will be made. 4 treatment groups of 6 will be created and 0.03ml AME treatment will be applied in variable doses. A control and SHAM group without treatment will be created from 8 rabbits. Clinical PVR staging will be performed, and sacrification and enucleation will be performed at the end of the 1st month. Growth factor expression will be examined in retinal tissue samples by ELISA method. It is predicted that the development of PVR will be prevented in the treatment groups in correlation with the dose, and the levels of growth factors associated with PVR will be lower than the control group. Demonstrating that AME can be used intravitreally in the PVR model for the first time will shed light on the treatment at an international level and may lead to other clinical studies.
