Evaluation of Sarcopenia by Adding Anthropometric Measurement to the SARC-F Questionnaire
Project no. 163 · Status: Kapanmış · 2018
Summary
OBJECTIVE: The aim of this study is to examine the performance of determining the sarcopenia by anthropometric measurements (mid-upper arm circumference and calf circumference) added to the SARC-F questionnaire developed as a screening tool for the risk of sarcopenia in the community-dwelling older adults. METHODS: The risk of sarcopenia of the individuals over 65 years of age who applied to the Geriatrics Department of Ege University Medical Faculty Hospital Internal Diseases Department. was determined by the SARC-F questionnaire, muscle mass was established by bioelectrical impedance analysis, muscle strength was assessed by handgrip strength, physical performance was assessed by a 4-meter walking test and presence of malnutrition was assessed with a MNA-long form. For the diagnosis of sarcopenia; old (EWGSOP 1) and revised (EWGSOP 2) diagnostic criteria of Sarcopenia Study Group in Elderly Individuals of the European Union Geriatric Medicine Association were used. New parameters were obtained by adding calf circumference (SARC-CC) and mid-upper arm circumference (SARC-MUAC) measurements were added separately and together (SARC-CC-MUAC) to the SARC-F. For the calf circumference cut-off points of <31 cm and <33 cm andfor the mid-upper arm circumference cut-off points of <25 cm and <31 cm were usedfor the sensitivity and specifity analyzes. RESULTS:We included 190 cases with a mean age of 73.3 ± 6.1 years, 30.5% were women and 69.5% were men. Fourty percent of the cases have at least one chronic disease, 36.3% of them have two, 8.5% of them have three and more chronic diseases. When classified according to the nutritional status determined using MNA-long form, 3.8% of women and 5.2% of men had malnutrition (p=0.004). Median SARC-F score was 2 (min 0-max 9) and 24.4% of the cases had sarcopenia risk according to SARC-F. Sarcopenia prevalence was 8.9% according to EWGSOP 1 and 4.7% according to EWGSOP 2. When the sarcopenia is evaluated by EWGSOP 1 criteria, the sensitivity of SARC-F was 41,2%, specifity was 84,9% and the AUC value was 0,695 (p<0,05). According to EWGSOP 2 criteria, the sensitivity of SARC-F was 77.8%, specifity was 43.7% and the AUC value is 0.594 (p>0,05). When calf circumference is added to the SARC-F, the sensitivity for SARC-CC 31 cm and SARC-CC 33 cm, respectively, were 77.7%, 66.7%; specifity were, 44.8, 89.5 and AUC values were 0.603 (p>0,05), 0.720 (p>0,05). When mid-upper arm circumference is added to the SARC-F, the sensitivity for SARC-MUAC 25 cm and SARC-MUAC 31 cm were 66.7%, 88.8%, specifity were 7.2%, 46.9% and AUC values were 0.511 (p>0,05), 0.639 (p>0,05); respectively. SARC-CC 33 cm-MUAC 31 cm has the highest positive predictive value (27,8%), negative predictive value (96,7%) and AUC (0,786; p<0,05) among the new variables created when CC and MUAC are added to the SARC-F. CONCLUSION:As a result of our research the prevelance of sarcopenia compared to EWGSOP 1 was approximately 2 times higher than that of EWGSOP 2. When the sarcopenia was evaluated according to the EWGSOP 2 criteria the sensitivity of SARC-F was higher than to the EWGSOP 1. Among the parameters with the highest value was observed when using the cut-off points <33 cm for CC and <31 cm for MUAC.
