Background: Following the 2018 classification, the European Federation of Periodontology (EFP) developed Clinical Practice Guidelines (CPGs) for the management of periodontitis and peri-implant diseases. To date, no clinical practice guidelines have been developed for non-dental biofilm-induced gingival diseases and conditions (NBIGD) or for gingival enlargement. Whilst these conditions are less common/rare, they present in general and specialist practice settings and there is therefore a need to develop such a guideline. Aim: To develop an S2k-level CPG for the management of NBIGD and conditions and gingival enlargement. Methods: This S2k-level CPG was developed by the EFP, following methodological guidance from the Association of Scientific Medical Societies in Germany and, where applicable, the Grading of Recommendations, Assessment, Development and Evaluation (GRADE) process. A rigorous and transparent process included: synthesis of relevant research in two specifically commissioned systematic reviews; evaluation of the quality and strength of evidence; formulation of specific recommendations; and a structured consensus process involving leading experts and broadly based stakeholders. Results: The S2k-level CPG for management of NBIGD demonstrates that due to their rarity, heterogeneity, complexity and the paucity of randomised controlled trials, NBIGDs should be managed within specialist centres, with urgent/immediate referral for suspected malignancies. Treating biofilm-induced inflammation is beneficial in most cases. Management of gingival enlargement is context-dependent and should be medical in nature, with surgery reserved for non-responsive cases. Conclusion: The present S2k-level CPG informs clinical practice, health systems, policymakers and, indirectly the public, on the most effective interventions in the management of NBIGDs and gingival enlargement.
