Introduction: Obstructive sleep apnea syndrome is associated with increased risk of metabolic and cardiovascular diseases. Positive airway pressure (PAP) treatment effect on these diseases’ biochemical findings is contentious. We therefore evaluated the change in Hb1Ac and lipid profile after short-term treatment with PAP. Method: The study population consisted of patients with obstructive sleep apnea syndrome (OSAS) diagnosis after polysomnography in university hospital and used 3 months PAP treatment from March 2012 to October 2019. Patients’ baseline and 3rd month of treatment Total Cholesterol, LDL Cholesterol, LDL Cholesterol and triglyceride levels were assessed Results: The Study population consisted of 913 OSAS patients, [69% were male, mean age was 53 (18-84) ]. Mean body mass index (BMI) was 33.27±6.48 kg/m2, Epworth sleep score was 10.28±5.56 and Apnea hypopnea index was (AHİ) 47.02±26.09/h After short term use of PAP treatment, there was a statistically significant change in total cholesterol (-3.14±30.75, p=0.003), LDL cholesterol (-2.57±0.97, p=0.008) and HbA1c (-0.07±0.79, p=0.01) There was no relation with metabolic parameters’ change and OSAS severity or duration of daily use of PAP after short term treatment. HbA1c reduction (-0.24±0.89, p=0.006) and HDL cholesterol increase (+1.29±3.86, p=0.002) were observed in patients with a concomitant weight reduction more than 5 kgs Conclusion: In conclusion, even after short term use of PAP treatment affects glycemc control and lipid profile postively. This effect was more prominant in patients with a concomitant weight loss more than 5 kgs
