Özet
Malign melanomda lenfosintigrafi ve intraoperatif gama prob ile sentinel lenf nodu saptanması hastalık yönetiminde oldukça büyük öneme sahip olan kabul görmüş bir uygulamadır.Gelişen teknolojiye paralel olarak son yıllarda, sentinel lenf nodu saptamada SPECT/BT gibi hibrid görüntüleme yöntemleri ile intraoperatif gama kamera ve hibrid detektörler de kullanıma sunulmuştur.Bu uygulama kılavuzunun amacı Malign melanom halen dünya çapında en öldürücü kanserler arasında olup, insidansı giderek artmaktadır.Malign melanomda lenfosintigrafi ve intraoperatif gama prob ile sentinel lenf nodu saptanması hastalık yönetiminde oldukça büyük öneme sahip olan kabul görmüş bir uygulamadır.Sentinel lenf nodu biyopsisi hastalığın nodal evrelemesini gösterir ve sentinel lenf nodunun invaze olup olmaması en önemli prognostik faktördür.Gelişen teknolojiye paralel olarak son yıllarda, sentinel lenf nodu saptamada SPECT/BT gibi hibrid görüntüleme yöntemleri ile intraoperatif gama kamera ve hibrid detektörler de kullanıma sunulmuştur.Bu uygulama kılavuzu, temel olarak nükleer tıp hekimlerine malign melanomda sentinel lenf nodu saptama yöntemleri hakkında bilgi vermek ve güncel literatür bulguları ışığında, ülkemiz şartlarında olabildiğince optimum şartlarda uygulama yapabilmeyi sağlamak hedefini taşımaktadır.Anahtar Kelimeler: Malign melanom, sentinel lenf nodu, nükleer tıpMalignant melanoma is still among the most fatal cancers worldwide with increasing incidence.Lymphoscintigraphy and gamma probe guided sentinel lymph node detection is an established procedure which has a great impact in disease management of malignant melanoma.Sentinel lymph node biopsy determines the nodal stage of the disease and invasion of the sentinel lymph node is the most important prognostic factor.In parallel to the latest technological developments in the recent years, novel hybrid imaging modalities such as SPECT/CT as well as intraoperative gamma cameras and hybrid detectors have been introduced.This procedure guideline is aimed to inform especially nuclear medicine physicians about the methods for detection of sentinel lymph nodes in malignant melanoma patients as well as to provide the state of art technique suitable for local standards in accordance with the current literature.
