Özet

e12653 Background: Breast cancer (BC) has unique clinical and biological variables in young and elderly patients. BC in young individuals tends to behave more aggressively with subsequent unfavorable outcome. Conversely, elderly pts usually have a more indolent BC course. We aimed to analyze the effect of young and older age on the natural course of BC in Turkish pts. Methods: Pts presenting to medical oncology clinics were prospectively registered after October 2012 in 28 centers. Patients were stratified according to age: young ≤ 35 yrs and elderly ≥ 65 yrs. The association of age with patient demographics and pathologic characteristics were evaluated. Results: 13749 pts (128 males [0.9%]) were registered. Median age was 53 yrs (20-93). Pts ≤ 35 and ≥ 65 constituted 4.2% and 20.9% of the whole population, respectively. Rate of male BC was tripled in pts ≥ 65 yrs compared to the ones < 65 yrs of age (2% vs 0.7%, p < 0.001). Histopathology and the grade of the tumors were similar. Stage distribution was also similar in pts ≥ 65 vs < 65 (stages I-IV in elderly: 17.8%, 50.6%, 23% and 8.5%,respectively, as compared to pts < 65: 17.1%, 48.5%, 26.5% and 7.9%, respectively). ER positivity was higher in pts ≥ 65 vs < 65 yrs (77.6% vs 75.2%, p < 0.001) with similar PR positivity rates. In contrary, Her2 was positive in a greater proportion of patients < 65 yrs compared to pts ≥ 65 (28% vs 21.7%, p < 0.001). Distribution of BC molecular subgroups was similar. Analysis of pts < 35 yrs revealed similar distribution of gender (p = 0.272) and disease stage (p = 0.571). But, proportion of grade 3 pts were higher in pts < 35 yrs (37.8% vs 30.0%, p = 0.005). Similarly, pts < 35 yrs tended to have negative ER (27.4% vs 24.3%) and negative PR (32.7% vs 28.7%) and amplified Her2 (36.5% vs 2.6%) (p < 0.001). Moreover, frequencies of Her2-enriched type (12.3% vs 8.8%) and triple-negative type (13% vs 11.5%) BC were higher in pts < 35 yrs of age (p = 0.016). Conclusions: Our results confirm previous observations reporting more aggressive tumor biology with higher rates of ER/PR negativity and Her2 amplification associated with higher tumor grade in young pts.
