Özet

Background: Idiopathic pulmonary fibrosis (IPF) has significant pathogenetic similarities with lung cancer, the latter characterized by PD-L1 expression as a predictive marker for immunotherapy. Aim: To examine the potential of serum PD-L1 (sPD-L1) as a prognostic marker in patients with IPF. Method: The study group was patients in the European MultiPartner Idiopathic Pulmonary Fibrosis Registry (EMPIRE) Türkiye cohort. A descriptive analysis was performed using the chi-squared test. Survival was analyzed using Kaplan-Meier / Cox methodologies. Results: Our cohort consisted of 636 patients with IPF. 54 with lung cancer and 28 with other cancers were excluded from this analysis of sPD-L1, leaving 554 as the subject of this study. The median age at presentation was 67 years (range 29 to 97), 72% were male. Out of 554 patients, 86% received antifibrotic treatment (nintedanib 32%, pirfenidone 54%), 48% of them died during the course of the study. The median sPD-L1 was 53 pg/ml (range 8 to 315). On Cox regression analysis with sPD-L1 as a covariate, sPD-L1 was a significant predictor of survival, with a p value of 0.019. For each unit increase in sPD-L1, there was a 0.5% increase in death risk (by HR). Conclusion: sPD-L1 is easily quantifiable, and it has potential as a prognostic marker in IPF. erj;66/suppl_69/PA5085/F1 F1 F1
