Summary
The focus of this study centered on the effectiveness of failure mode and effect analysis (FMEA), which is a reliability analysis technique aimed at zero failure in quality. FMEA is a specific methodology to evaluate a system, design, process, or service for possible ways in which failures (problems, errors, risks, concerns) can occur. These risks can be measured by reliability engineering and/or statistical analysis. The application data are based on the observed failure frequencies related to the potential failure modes associated with the product, which is a gear synchromec machine CNC-01. Only the occurrence degree has been determined with the Poisson process approach for FMEA methodology. The severity degree and detection degree are intuitively determined with the abstract approach, and the risk priority number indications have been calculated and the priority of improvement activities has been defined by a Pareto chart.
