Background: Excessive daytime sleepiness (EDS) is a key symptom and potential predictor of cardiometabolic disease in OSA. However, the presence of the sleepy phenotype and its association with comorbid conditions is less well characterized in mild OSA. Aims and objectives: To identify the characteristics and the cardiometabolic risk of the sleepy phenotype in mild OSA patients in the ESADA cohort. Methods: We investigated 10,501 patients who completed the Epworth sleepiness scale (ESS). The predictive value of the sleepy phenotype (EDS-ESS>12) on prevalent cardio-metabolic disease was investigated after adjustment for confounding factors. Results: Mild OSA patients with EDS were younger (50 vs 53yrs, p<0.001) had higher BMI (31 vs 29 kg/m2, p=0.002), increased proportion of females (37 vs 34%, p=0.02) and higher prevalence of diabetes (12 vs 10%, p<0.001) and lower percentage of hypertension (33 vs 38%, p=0.004) compared with those without EDS. Moreover, the sleepy phenotype predicted hypertension in the group of non-obese, older females [OR (95% CI) 3.148 (1.176–8.423), p=0.02], Cardiovascular Disease (CVD) in the group of non-obese, older males [OR (95% CI) 2.057 (1.023–4.138), p=0.04], and Diabetes [OR (95% CI) 1.679 (0.654–4.312), p=0.04] in the group of obese, older females. Conclusion: Our results suggest that mild OSA patients with the sleepy phenotype seem to be younger, with higher BMI and predominantly females with susceptibility to develop hypertension, CVD and diabetes in specific subgroups of patients.
