Summary

A new therapeutic approach to left ventricular hypertrophy

Öz (İngilizce):In general drugs that influence renin angiotensin aldosterone system (RAAS) has been shown to reduce left ventricular hypertrophy (LVH). We investigated the effects of Losartan -K+ an angiotensin II (Ang II) receptor antagonist, on LVH, plasma renin activity (PRA), serum aldosterone levels and blood pressure levels.37 patients, M/F was 17/20, aged 35-70 years with a systolic blood pressure (SBP) of 140 mmHg or more and/or diastolic blood pressure (DBP) between 90 to 115 mmHg were included in the study. All had echocardiographic evidence of LVH. Following 2 weeks washout period baseline echocardiography was performed to get left ventricular dimensions. PRA and serum aldosterone levels were measured simultaneously. After 6 months of treatment with 50 mg ofLosartan daily, the parameters mentioned above were controlled again, there was a significant decrease in SBP, DBP and serum aldosterone levels (P<0.001), but a moderate decrease in PRA, left ventricular mass (LVM) and left ventricular mass index (LVMI) (P<0.05). In conclusion, we can say Ang II receptor blockade may be expected to provide more comprehensive blockade of the trophic actions of Ang II.
