Evaluation of 116 Rectum Carginoma Patients Treated With Postoperative Radiotherapy

Özet
HAYDAROGLU. Between J anuary.l989~Decembeı: 1994, postoperathre radiotherapy wa.s given.116 patients with stage B2, B3, Cl, C2, C3, redum cardnoma, according tomodified Astler CoUer staging system .. Age of patients :ranged from 18 to 82 (median 51.5.);73 (2.9%).weremen, 43 (37.1%) were women; 54 (46.6%)of patients were staged as B2•3 and62 (53.4"'/.,)as.cı~3fifty•one (44%) pal:ients underwent abdominoperineal resedion, 36 patients (31~/o) low anterior resedion, and 29 patients (25%) anterior resedion.Radiotherapy was giverı.by Co 60teleth~rapy or.Hneeır-accelerato:r (6s 25MV) on prone position to a total dose of 50,4-54 Gy (median 54 Gy) with 1,8 Gy daily fractions During radiotherapy, 5 pathmts (4.3"/.,)developed diarrhea grade 2, which caused an interruption of 3-10 days.Late complication § such as bowel obstnıction or necrosis we:re seen in 10(8.6%)patients.Eighteen patients (15.3%) had developed local recurrence aıtd 22 (19.9%}patitents had distant m~ tastas is; the time to local ırecurrence was 1148 monl:hs (median20.5months).The fiv~year local, distant matastasis free survival, disease free smvival and overall smviv;ıl rates were 81.0%, 73.0%, 60.6% and 72.5% respedively.The loaıl control ırates were influenced by the bımour-analverge distance (p=0.049) and the surgical methods (p=0.007), the disease free smvival rates by the tmnom-arı.alvergedistance (p=0.035), the su:rgical mefuods(p=0.35),the stage (p=0.048)and the histopamological diagnosis (p=O.Ol7), wich wereevaluated by univariantanalysis.Multivariate analysisof Cox regressioumodel di d not reveal any independent prognostic fador.It was obtained, that the histopatlıology (p=0.052)was the independent prognostic fador.for disease free survival, the stage (p=0.025)and hist.opathology(p=0.014) for distant metastasis free sw:vival, md the sex (p=0.042) for the overall suıvival.
