These patients stay in the waiting list longer than others, and face with cardiovascular events. Starting from early ages, obesity predisposes to post-transplant adverse cardiac, renal, infectious, biliary and surgical complications, and however no adverse effect on patient and graft survival in long term. Fatty liver, decreases the quality of the donor pool, and creates a need to use more marginal donors. Except for advanced disease, weight reduction improves the risk profile. At the post'transplant period with the effect of immunosuppressives, gaining weight and other metabolic factors, recurrent and de novo steatohepatitis may develop that predispose to diabetes, renal and cardiac problems. Nutritional, medical and surgical weight reduction therapies under close surveillance have positive effects on prognosis.
