Introduction: The aim of this study was to evaluate the prevalence of latent tuberculosis infection (LTBI) in our interstitial lung disease (ILD) patients. Methods: Patients with ILD were tested by using a commercial test QuantiFERON®-TB-Gold In Tube (QFT) at the time of initial diagnosis, before any immunomodulatory drug was initiated. Those patients with a historical or active TB were excluded, as were any with an equivocal QFT result. They were compared with 272 control cases who had a normal chest x-rays or thoracic CT, and no history of TB. Comparisons were classified by age (<45, 45-64, ≥65). Results: The study population was 372 patients with an established diagnosis of ILD of various forms and 272 control cases. The cases of ILD were 197 men and 175 women, with a median age at presentation of 60 years. Most had IPF (46%), sarcoidosis (23%) or CVD-ILD (14%), the remaining 17% having a variety of other forms of ILD. In the control group, the rate of QFT positivity rises with age. The rates of positivity rise with age in ILD patients generally, and in CVD-ILD and IPF specifically, paralleling the rates seen in the control group. The rate of QFT positivity does not rise with age in patients with sarcoidosis. Conclusion: Sarcoidosis may cause an immune paresis preventing the acquisition of QFT positivity. Further investigation is required to determine whether there is a group of older patients with sarcoidosis and LTBI who have a false negative QFT and would be at risk of activation of their TB if treated with anti-TNF-alpha agents.
