Background: Available studies show an overall poor correlation between sleep characteristics and symptoms profile in mild obstructive sleep apnoea (OSA) patients. Aim of the study was to explore the relationship of daytime sleepiness, as assessed by Epworth Sleepiness Scale (ESS), with anthropometric, sleep and metabolic characteristics in mild OSA patients (apnoea-hypopnoea index [AHI] 5-15/h) in the ESADA multicenter cohort involving over 20 academic sleep centers. Methods: Data were collected on anthropometrics, ESS, metabolic and sleep variables obtained from overnight polysomnography. Results: A total of 2068 adults (63.6% males) with mean AHI 9.5±2.9/h, age 50.7±12.5 years, BMI 29.1±5.7 kg/m2, and ESS 9.2±5.1, were included in this analysis. ESS was correlated negatively with age (r=-0.115, p<0.001) and positively with BMI (r=0.062, p<0.001) as well as with neck (r=0.032, p=0.033), waist (r=0.033, p=0.031) and hip (r=0.047, p=0.002) circumferences. No correlation with AHI was observed (p=0.360). Among sleep variables, ESS correlated with total sleep time (r=0.062, p=0.005), sleep efficiency (r=0.101, p<0.001), N1 duration (r=-0.069, p=0.002), lowest SpO2 during sleep (r=-0.040, p=0.007) and oxygen desaturation index (r=0.035, p=0.021). Finally, no correlation between ESS and any of the recorded metabolic parameters was found. Conclusions: In mild obstructive sleep apnoea patients, daytime sleepiness correlated with indices of nocturnal hypoxia, sleep, and obesity, but not with AHI or metabolic parameters.
