Background: COPD is a significant public health problem. Misuse of metered dose inhalers (MDI), a common COPD treatment, is prevalent. Illness perceptions and reactions influence health behaviors and medication adherence. Objective: This study examined the relationship between COPD patients' MDI use skills and their perception and reaction to COPD, as well as variables influencing these perceptions and reactions. Method: Data were collected from 184 hospitalized COPD patients using MDI. The patient information form, MDI skills chart, and COPD Perception and Reaction Scale (COPD PRS) were used. Correlation analysis assessed the relationship between MDI skill scores and COPD PRS scores. Regression modeling identified explanatory variables for COPD perceptions and reactions. Results: 79% of patients were male, with a mean age of 64.77±9.59 years, and 59.1% had systemic disease. The mean total score (MTS) of MDI use skill was 3.26±1.43. MTS of the COPD PRS was 77.22±19.45. A negative, moderate relationship was found between MDI skill and COPD PRS scores (r=-0.693; p<0.000). MDI skill score (β=-0.358; p<0.001), age (β=0.205; p=0.005), COPD duration (β=0.188; p=0.004), and systemic disease presence (β=0.166; p=0.004) significantly explained COPD PRS scores. Conclusion: COPD patients continue to make errors in MDI use and have negative perceptions and reactions to the disease. Regular assessment of COPD perception and reactions, as well as personalized educational interventions, is recommended.
