Özet

-ABSTRACT

Objective: Hypofractionated radiotherapy (HRT) is known to have advantages such as reduced accelerated repopulation of tumor

clonogenic cells, reduced cost of treatment as well due to the fewer fractionations, and reduction of hospitalization time. In our study,

we aimed to investigate the effects of HRT on local control, survival, early, and late toxicities of treatment in locally advanced NSCLC.

Material and Methods: Between August 2003 and January 2005, twenty-four locally advanced (Stage III) NSCLC patients, who had

a comorbid disease, inoperable, or did not accept surgical/systemic treatment, were included in the study. Curative radiotherapy

was applied to cases in 4 Gy fractions, 5 fractions/week, and in total 12 fractions.

Results: All patients were male with a median age of 72.5 years (range 46–79). The median KPS was 80% (range 60–90%). Fifteen

cases (62.5%) had squamous cell carcinoma (SCC) pathology. The first symptoms were cough, pain, dyspnea, hemoptysis, dysphonia

and dysphagia, respectively. The symptomatic response was obtained in 100% of patients with hemoptysis, 73% of patients

with pain, and 60% of patients with dyspnea. Radiological tumor response evaluation was conducted in 21 patients. There were 6

(25%) patients with stable disease, 14 (67%) patients with partial, and 1 (5%) patient with complete response. The median overall

survival (OAS) time was 11.4 months (range: 6–30). OAS rates after 12 and 24 months were 34.8% and 8.7%, respectively. İn terms

of prognostic factors affecting OAS, only hemoglobin value (p=0.029) and SCC histology (p=0.022) were defined as significant

prognostic markers among parameters such as T and N stage, histopathology, hemoglobin, and pre-treatment FEV1 and DLCO values.

Acute grade-I/II esophageal toxicities were observed in 6/12 and pulmonary toxicities were found in 9/2 patients, respectively.

Late toxicity was evaluated in 20 patients. There were 6 grade-II, 1 grade-III radiation fibrosis, and 1 grade-I esophageal fibrosis.

Conclusion: Symptomatic response and OAS rates using HRT in patients with locally advanced NSCLC were comparable with

other fractionation schemas and can be offered in daily routine practices in selected patients.
