Introduction: Chronic rejection is a condition characterized predominantly by severe bile duct damage. In this study, we aimed to determine whether hepatic mast cells are involved in pathogenesis of chronic rejection after liver transplantation. Methods: Biopsy specimens from normal livers (n = 5), transplanted livers with end-stage chronic rejection (n = 5), and transplanted livers with acute cellular rejection (n = 26) were studied. Biopsy specimens were stained histochemically for mast cells with toluidine blue. The mean number of mast cells per portal tract was calculated from the total number of mast cells occurring in portal tracts and number of portal tracts containing mast cells. The degree of inflammation in each biopsy specimen was graded from 0 to 3 (0, no or minimal inflammation; 1, mild inflammation; 2, moderate inflammation; 3, severe inflammation). Comparison between groups was made using Mann-Whitney unpaired two-tailed tests. Results: Mast cell number was significantly increased in the chronic rejection group compared with both controls and acute cellular rejection group. The percentage of portal tract containing mast cells was significantly higher in chronic rejection specimens than control biopsy samples, as was the average number of mast cells per portal tracts. Tissue mast cells frequently were seen surrounding damaged bile ducts in inflamed portal tracts and it was correlated positively with degree of inflammation but not degree of cholestasis. Increasing grades of rejection were also associated with progressively more mast cells per portal tract in acute cellular rejection group (r=0.57, p=0.01) but there was not significant difference than control group respect of portal tracts with MCs. Conclusion: Mast cells may play a role in the chronic rejection response to the liver allograft.
