Background: Fitusiran is a subcutaneous, investigational siRNA therapeutic that lowers antithrombin (AT) to increase thrombin generation and rebalance hemostasis in people with hemophilia A or B (PwHA/B), irrespective of inhibitor status. Phase 3 trials (ATLAS-INH [NCT03417102]; ATLAS A/B [NCT03417245]; ATLAS-PPX [NCT03549871]; ATLAS-OLE [NCT03754790]) demonstrated clinically meaningful bleed control with fitusiran. To treat bleeding events during fitusiran prophylaxis, bleed management guidelines (BMG) recommend reduced doses and frequencies of clotting factor concentrate (CFC)/ bypassing agents (BPA) as there is a potential for enhanced thrombin generation with concomitant use.
