Özet

Background. The incidence and nature of cardiac involvement in Behçet's disease have not yet been clearly documented. Transthoracic echocardiography (TTE) has been used for detecting cardiac abnormalities in previously published studies, however, the diagnostic role of transeosophageal echocardiography (TEE) in Behçet's disease has not yet been established. Aim. To assess the role of TEE for complete definition of cardiac involvement in this patient population. Methods. We performed TTE and then multiplane TEE to evaluate cardiac involvement in 35 patients with Behçet's disease (9 women and 26 men, mean age 38±12 years) and 30 normal subjects who were selected and matched with the patients for gender, age and body surface area. Results. Sinus Valsalva and ascending aorta diameters were found significantly larger in patients with Behçet's disease than controls (p<0.001 and p<0.0001, respectively). Aneurysmatic dilatation of sinus of Valsalva and ascending aorta were detected in 28% and 48% of patients (p<0.001 and p<0.0001, respectively). Patent foramen ovale was observed in 28% of patients and 10% of normal subjects (NS). Interatrial septum aneurysm was found in 31% of patients with Behçet's disease and 6% of controls (p<0.026). Compared with controls, patients had significantly more frequently mitral valve prolapse (25% versus 3%, p<0.033) and mitral regurgitation (40% versus 6%, p<0.004). Conclusions. Echocardiographic evaluation of patients with Behçet's disease enables early detection of potentially harmful cardiac abnormalities. An interatrial septal aneurysm, dilatation of the proximal aorta, mitral valve prolapse and mitral regurgitation are the common findings of cardiac involvement in Behçet's disease. Transesophageal echocardiography has additional advantage in detection of these abnormalities.
