Introduction: Positional obstructive sleep apnea (POSA) is a subgroup of sleep apnea in which respiratory events are more common in supine position. The aim of the present study was to investigate the clinical and polysomnographic features of POSA and compare with non-POSA patients in a large population. Methods: The data of 5098 patients underwent polysomnography at Ege University Hospital were analyzed. After excluding the patients with apnea-hypopnea index (AHI)<5/h and lack of position data, 2389 patients were included in the study. Supine AHI more than 2-fold of non-supine AHI was defined as POSA. POSA patients were divided as supine predominant (spPOSA) whose non-supine AHI≥5/h, and supine isolated (siPOSA) whose non-supine AHI<5/h. Results: Out of 2389 patients, spPOSA and siPOSA had a prevalence of 37.0% and 21.6%, respectively. There was a significant male dominance in spPOSA (74.5%) and siPOSA (71.8%) patients (p<0.001). siPOSA patients had lower mean age (48.7, 51.9 and 52.8 years, respectively), AHI (15.9, 35.1 and 55.5/h, respectively) and body mass index (29.0, 31.5 and 34.8 kg/m2, respectively) than spOSA and non-POSA patients (all p<0.001). Non-POSA patients had more common coronary artery disease (p=0.013), hypertension (p<0.001) and diabetes mellitus (p<0.001) compared to those with POSA. In siPOSA patients, sleepiness was less and mild OSA (57.1%) was more frequent. In addition, auto-adjusting PAP therapy was preferred more frequently in POSA than in non-POSA patients (p<0.001). Conclusion: Positional sleep apnea patients, especially isolated positional ones, are young, less obese, non-sleepy men with mild OSA and less comorbidities. These patients should be evaluated as a different phenotype of OSA.
