Abstract 2 Several lung disorders can occur as a consequence of chronic liver disease. Hepatopulmonary Syndrome (HPS) is a triad cirrhosis, pulmonary vascular dilatation and abnormal arterial oxygenation. The aim of the study was to investigate pulmonary complications in 18 children with cirrhosis (12 boys, 6 girls, mean age 11 yr, range 10 mo-17 yr). Cirrhosis were determined by biochemical, serologic, histopathologic, sonographic and endoscopic investigations. In order to evaluate pulmonary complications chest radiography (18), pulmonary function tests (18), arterial blood gas analysis (18), ventilation-perfusion (VP) scintigraphy (14), DLCO (5) and contrast echocardiography (ECHO) (18) were used. To determinate pulmonary arteriovenous (AV) shunts, 20% galactose solution was used as a contrast material during ECHO. In all patients alveolar-arterial gradient was calculated. (Table) In this study, HPS triad was established in 4 patients (22%). Five patients (28%) were hypoxemic. Four of hypoxemic patients and another one (28%) had AV shunts and 4 (44%) of these were hypoxemic also. Three of the patients with HPS had liver transplantation in which the ECHO findings regressed in all of them postop on 6th month. None of them had pulmonary hypertension. Conclusions: HPS is a serious and important complication of cirrhotic children. It will be appropriate to evaluate these children with noninvasive method as constant ECHO especially if digital clubbing is present. HEPATOLOGY
