Background: Abundant evidence supports an association between Idiopathic Pulmonary Fibrosis (IPF) and lung cancer development. Data on diagnosis and management of patients with IPF and lung cancer are still scarce. Aim: To present clinical features of patients with IPF and lung cancer. Patients and Methods: This was a retrospective, multicenter study enrolling 3178 patients with IPF in Europe between 1/1/2019 and 31/1/2021. Results: We identified 324 patients with IPF and lung cancer [prevalence: 10.2%, median age: 71 years (95% CI: 70 to 71), males: 90.4%, mean FVC%predicted ±SD: 76.9 ± 20.1, mean DLCO%predicted ± SD: 45.8± 16.9]. Non-small cell lung cancer was identified in 73.5% of patients (squamous: 33.3%, adenocarcinoma: 26.9%, NOS: 13.3%), while 17.4% and 9.1% of patients were diagnosed with small cell lung cancer and other types of cancer, respectively. Lung cancer was diagnosed synchronously with IPF in 41.8% and post IPF diagnosis in 45.4% of cases with a median latency time of 28.1 months (95% CI: 20.3 to 32.7). The tumor was otherwise operable in 46.6% of cases (IA: 19.1%, IB: 13.5%, IIA: 2.2%, IIB: 10.1%, IIIA: 10.1%, IIIB: 11.2%, IIIC: 2.2%, IVA: 19.7%, IVB: 11.8%). Antifibrotics were prescribed in 50.9% of patients. Chemotherapeutic regimens other than nintedanib were applied in 46.2% of cases, while 28.3% of patients underwent surgery. Median survival was 21.4 months (95% CI: 18.8 to 26.5) from the time of lung cancer diagnosis. Conclusions: IPF represents a risk factor for lung cancer development. Disease management remains heterogeneous. Randomized controlled studies on the management of patients with IPF and lung cancer are greatly anticipated.
