Introduction: Excessive daytime sleepiness (EDS) is a prominent treatable trait in obstructive sleep apnoea (OSA). The current study analysed if referral pattern over time confirms EDS as key symptom of OSA. Method The European Sleep Apnea Database (ESADA, an ERS supported CRC) collects data from patients with suspected sleep apnoea at 40 sleep sites in 18 countries. Anthropometrics, Epworth Sleepiness Scale (ESS) score, and sleep test results were analysed in 37,615 subjects (mean age 53±13y, Body Mass Index (BMI) 31.9±6.7kg/m2, ESS 9.3±2.3, Apnoea Hypopnea Index (AHI) 31.8±21.6) over time (2008-2024). Linear mixed regression analyses with year as dependent variable and country/region as random factor assessed changes over time in EDS. Results: Mean age (+2.5 years), BMI (+1 kg/m2) and AHI (+10 n/h) increased, whereas ESS decreased (-2 units) over time (Figure). Adjusting for factors listed above, ESS score declined by 0.124 (SE 0.006) each year (p<0.001). Addressing confounders and country, the population marginal means for ESS score were 10.4 (SE 0.16) in 2008 and 9.0 (SE 0.44) in 2024 (p>0.001). Hypertension diagnosis did not affect the model. erj;66/suppl_69/PA1085/F1 F1 F1 Conclusion: EDS complaints were reduced over time in patients referred for OSA evaluation across Europe. Our data indicate the need for better alignment of referral practice with current guidelines in OSA management suggesting “symptomatic OSA” as the main treatable trait.
