Özet

A modern dental rehabilitation method integrates fixed dentures using the “All-on-4” concept for edentulous arches. Angled distal implants preserve crucial anatomical structures eliminating the need for a transitional period seen in delayed loading systems. These case reports present the 1-year follow-up of fixed dentures fabricated from metal-acrylic or monolithic zirconia superstructure following the All-on-4 protocol. All five patients were systematically healthy and edentulous. All implants were placed according to All-on-4 concept and multi-unit abutments were selected prior to the prosthetic phase. Three cases were restored with fixed acrylic superstructure. Acrylic restorations in both jaws had no complications in two cases, while the case with natural dentition in the opposing jaw experienced tooth detachment twice. The other two cases were restored with monolithic zirconia superstructure with either natural lower dentition or semi-attachment prosthesis in the antagonist jaw. They did not show any chipping or fracture while providing better aesthetics. 6th and 12-month follow-ups were done. In the literature although acrylic superstructures are a more affordable method of treatment providing a lighter superstructure, they are more prone to breaks, cracks and discolouration. In contrast, monolithic zirconia is more resistant to fractures, chipping, long-term wear while offering superior aesthetics. Satisfactory results have been achieved in all cases in terms of aesthetics, and function. While no chipping, fractures were observed in zirconia superstructure, acrylic superstructure resulted with tooth detachment, and some discolouration.
