A massive extrapleural haematoma secondary to blunt chest trauma is exceedingly rare and the recent report in this journal of such a case by V. G. Machin and O. J. Lau (Eur J Cardio-thorac Surg 1995; 9: 109-110) prompts us to record the injury sustained by a retired 90-yearold surgeon when he fell against a kitchen table. No medical treatment was sought. After a somewhat disturbed night he was found by his wife to be in a state of extreme collapse the following morning and sadly he died soon afterwards. The post-mortem examination revealed fractures of the 9th-1 lth ribs and a 1~ 1 extrapleural haematoma caused by injury to the intercostal vessels. He had taken 600-900 mg aspirin daily for several months to relieve the pain of his arthritis and the anticoagulant effect of this drug had undoubtedly contributed to the size of this haematoma.
