Oral mucosal pigmented lesions encompass a diverse spectrum of pathologies, ranging from benign manifestations to potentially malignant conditions. These lesions pose diagnostic challenges due to their varied presentations and etiologies. We present a case of a 16-year-old female with a dome-shaped brownish blue papular soft lesion in the right retromolar area. The patient presented with complaints of dental pain but had an unremarkable dental and medical history, no medication intake, and no history of trauma to the oral region. Clinical examination revealed a slightly elevated lesion with ill-defined borders and linear brown patterns resembling hair-like formations. Radiological evaluation showed an intact bony architecture. Differential diagnosis included melanosis, melanotic macule, various types of nevi, and melanoma. Complete excision of the lesion was performed under local anesthesia, followed by histopathological evaluation, confirming an oral compound nevus. Postoperative recovery was uneventful, with no complications reported during follow-up. Oral melanocytic nevi, though rare, require careful consideration in the differential diagnosis of oral pigmented lesions. The compound nevus subtype, characterized by nevus cells in both epithelial and connective tissue components, particularly manifests in young individuals. Diagnostic vigilance and thorough evaluation are necessary to differentiate nevi from potentially malignant lesions like melanoma. The compound nevus is an uncommon subtype of oral melanocytic nevi, often presenting in the retromolar region of young individuals. Surgical excision followed by histopathological examination is crucial for definitive diagnosis and exclusion of malignancy. Long-term follow-up is essential to monitor for any recurrence or malignant transformation.
