Tuba Kuvvet Yoldaş, Alev Atalay, İlkin Çankayalı, Kubilay Demirağ, Mehmet Uyar

INTRODUCTION: In the last century, advances in medical care and longevity have resulting in an

increase in the elderly population. In parallel with the eldery population, burn injury seen in old people

have been increasing. Burn injury; causes more serious injuries and high level of mortality in people

aged ≥65 years compared to the general population. Reduction in sensory/cognitive functions, slowing

reflexes, limited movements and which are due to physiology of elderly people increase the burn

incidence and the severity of burn. We retrospectively analyzed hospital stay, mortality and morbidity of

6 patients over 65 years of age, who followed up in 2018-2019 at Ege University Burn Intensive Care

Unit.

CASE: Six of the 50 patients we followed up at the burn intensive care unit in 2018-2019 were over the

age of 65. Admission criteria of burn intensive care are TBSA(Total body surface area) ≥ 20%, inhalation

injury, any electrical contact or chemical injury; elderly patients with burns; or burns to vital areas-face,

hands, feet or perineum. The means age of 6 patients was 71 years old(66-80). 4 patients were male,

2 patients were female. Burn causes were scald and flame in this patients. Patient’s percentages of the

burn damages between 6% and 90%(mean %29,6). 2 patients were TBSA<20%, 4 patients were

TBSA≥20%.Two patients had inhalation injury. The APACHE II mean score was 17.5, and the ABSI

mean score was 8.5. The incidence of sepsis was 50%, the mean hospital stay was 19.3 days. The

survival rate was 33%. The patients who developed mortality had comorbid diseases, 2nd and 3rd

degree burns, inhalation injury and sepsis decreased the recovery rate. Table 1 contains summary of

the patients.

DISCUSSION/CONCLUSION: Significant progress has been made in the treatment of burns in recent

years, but the improvement of health outcomes in elderly adults with burn injuries is much lower than

that of young patients(1). Burn injuries are the 4th leading cause of death in elderly patients. Muhammed

et al.(2) reported the mortality rate as 54.2% in 48 burn patients over the age of 60. Even if TBSA <20%,

inhalation damage increases mortality rates. Mabrouk et al. (3) reported the mortality rate as 70.9% in

patients with DM and CAD. Elderly patients have a weaker immune system post-burn infections. The

presence of comorbid diseases, deep and wide burn areas, inhalation damage and the addition of

infection increase mortality rates(4). In this patients who with comorbidities, TBSA 20%, inhalation injury

and sepsis resulted in mortality. Geriatric burn damage; is a trauma and it can be prevented. The best

way to avoid mortality caused by burn injury at geriatric patients is to avoid burns.

Keywords: Burn injury,Elderly, Mortality
