e20101 Background: The association between lymphocyte infiltration status and disease characteristics of melanoma and whether lymphocyte infiltration status is independently related to prognosis in non-metastatic cutaneous melanoma patients were evaluated. Methods: Medical records of non-metastatic cutaneous melanoma patients admitted to our clinic between 2000 and 2014 were retrospectively reviewed. All consecutive patients with available data regarding primary histopathological characteristics were included. Patients were grouped into “brisk infiltration” and “non-brisk or no infiltration” groups according to lymphocyte infiltration status of primary tumor. Disease characteristics and survival outcomes were compared among two groups. Results: Overall, 224 patients were included. Brisk infiltration was detected in 40 (17.9%) patients. Among brisk infiltration and non-brisk/no infiltration groups; median age (53 vs 52 years, p:0.536), gender distribution, and ratio of ulcerated lesions were similar. Brisk infiltration was associated with lower Breslow thickness (median 2.83mm vs 3.54, p:0.024), lower mitotic count (median 2 vs 7/mm2, p:0.015 ), less local lymph node positivity (13.5% vs 35.8%, p:0,009). In a median follow-up duration of 50 months, 13 out of 40 patients (32.5%) with brisk lymphocyte infiltration, and 96 out of 184 patients (52.2%) with non-brisk/no infiltration had disease recurrences. Median disease-free survival duration was superior in brisk infiltration group (68.2 vs 35.8 months, p:0.014); 2 and 5 year disease-free survival rates were 89.2% and 56% in brisk, whereas 61.3% and 42.6% in the other group, respectively. In multivariable analysis, when adjusted for tumor thickness ( ≤ 2 mm vs > 2 mm) and lymph node status (positive vs negative), lymphocyte infiltration (brisk vs other) was not significantly associated with disease-free survival (Hazard ratio: 1.63, 95% CI: 0.83 – 3.19, p:0.158). Conclusions: Our results suggest that presence of brisk lymphocyte infiltration of primary melanoma is associated with lower Breslow thickness, less lymph node metastasis and lower mitotic count.
