Summary
Foreword Recently, ECCO2-R has been shown to be a favorable rescue-treatment option in cases of resistant/retractable respiratory/hypercapnic acidosis. Aim: This study was planned to elucidate ECCO2-R treatment efficiency and effect on survival of ICU patients. Method: Patients’ HRs, admitted to Trakya University research hospital 10-bed capacity mixed type tertiary-ICU in between 1st Jan – 31st Dec 2017, were retrospectively analyzed. Results: Total of 12 ECCO2-R patients’ data out of 471 were included into the study, between. Patients’ (n:12, 7 male); mean SOFA 13±1 mean age was 54.9 (25-74), mean LOS 13.6 (4-29), pre-PaCO2 87.5 (58-158) and post-PaCO2 47.8 (22-65), pre-FiO2 68 (40-90) and post-FiO2 40 (21-50), pre-pH 7.22±0.19 and post-pH 7.35±0.10, and 9 were dead at follow up due to comorbidities. Patients’ pre-Vt and pre-FiO2 values significantly improved during and after ECCO2-R (p<0.05 and p<0.01 respectively). Compensated relative-high pre-HCO3 levels did not show an acute improvement after ECCO2-R. PaCO2 levels successfully lowered by 24hrs procedure (p<0.01), otherwise PaO2 levels did not change. Arterial pre-pH levels, all were resp-acidosis, were all improved and normalized (p<0.01). Serum urea, creatinine, albumin, CRP, electrolytes, hemoglobin, WBC, platelets levels was not fluctuated. Despite parameters were improved, statistical analysis did not show any significant improvement of survival or ICU LOS. Conclusion: ECCO2-R therapy successfully improved some vital parameters in patients with retractable hypercapnia / respiratory acidosis, on the other hand it did not affect patients’ survival, remained questionable yet.
